Catalogue page and half-title refer to this being Simon and Schuster published, at variance with binding and jacket. We suspect this page was inserted before the Simon and Schuster edition was prepared, but submitted to LoC for indexing prior to full publication date of Sept 1977. LoC have this book as published 1976. To further confuse things, the signature is dated Nov, 1978 and placed in Halifax, Woodroffe's birthplace (see images).
